Education: High school diploma or G.E.D. Necessary

Special Requirements: Must be 18 years of age. Must be able to lift 60 pounds. Will have, or obtain as soon as possible after hiring, a Colorado driver’s license. Will have, or obtain as soon as possible after hiring, first aid certification. Be present at trainings provided by Horizons. Essential Functions The Milestones Team Coordinator will work closely with the Milestones Day Program Coordinator (DPC) to operate a day program for individuals with intellectual and developmental disabilities in Steamboat Springs. In many respects, the Milestones TC is a resource for clients, Direct Support Professionals (DSP’s) and agency co-workers to communicate information and processes.

The Milestones TC will facilitate the following job tasks:

1. Scheduling: Assist Day Program Coordinator (DPC) with the following daily and weekly scheduling: a. Schedule weekly client activities and client work hours. b. Schedule split, smaller groups. c. Schedule 1:1 staff or volunteers during day program. d. Schedule staff to fulfill medical or therapy appointments during Day Program. Day Program Team Coordinator Job Description: e. Schedule vehicles, drivers, transportation: pick-up, drop-off, transportation to activities and work, if necessary, medical or therapy appointments. f. Schedule and assign shopping and cooking for staff and clients. g. Assist with Day Program organizational structure schedule, i.e., client chore schedule.

2. Training: a. Assist with training of new Day Program staff on paperwork and procedures; document training for personnel file. b. Assist in developing Day Program module with trainer, including detailed Day Program counselor job description and provide ongoing input for performance evaluation of Day Program staff. c. Assist in training for clients’ emergency procedures. Conduct regular fire drills and document in Emergency Procedure Book.

3. Day Program Needs: a. Supervise client daily activities and responsibility for the safety and well-being of clients. b. Teach self-sufficiency abilities and skills. c. Assist with evaluation of clients using Day Program Assessment. d. Assist with Day Program maintenance, i.e., everyday fix-it needs, problem solving and contacting Director of Administration for professional maintenance needs. e. Help maintain necessary supplies, materials and equipment for Day Program within budgetary guidelines. f. Maintain first aid supplies at site and in Day Program vehicles. g. Monitor community sites for environmental and programming quality, and inform DPC. h. Communicate with residential staff to coordinate activities, birthday parties and events.

4. Documentation: a. Ensure MAR’s are signed and documented. b. Ensure client Day Programs are implemented and documented. c. Ensure all events of an emergency nature or concern are documented on an Incident Report and are submitted by the next working day to DPC. d. Document client daily attendance. e. Ensure client daily progress notes are documented. f. Ensure client Day Program files are updated and complete. g. Assist DPC with appropriate PAR information. h. Assist with quarterly reviews.

5. Behavioral Management: a. Ensure the management of behavioral issues and follow-through at the Day Program site and community. b. Ensure client Behavioral Programs are implemented and documented. c. Communicate with residential staff regarding behavioral issues.

6. Other duties: a. Attend trainings when requested by DPC. b. Attend meetings when requested by DPC. c. Assist with interviewing for SIS Assessments. Day Program Team Coordinator Job Description: d.
