Salary: $55,000 - $60,000

To advance issues around health equity in our Spanish speaking community, the Health Partnership seeks a Bilingual/Bicultural Promotor/a who is a well-organized, self-starter with excellent communication skills. The successful candidate will exhibit the following:

Supervision: The Promotor/a is based out of the Steamboat or Craig office and supervised by the Care Team Director.  

The Role: The Promotor/a works with Spanish-speaking individuals and families to support them in getting connected to the resources they need to thrive. This position also identifies community needs, raises awareness of social determinants of health issues (physical and mental health barriers, health insurance, affordable housing or transportation) and participates in the development, communication, implementation and expansion of community education and access to programs. The Promotor/a also works collaboratively with a variety of community partners to ensure equitable access to resources in the community.  

This position requires strong communication, clerical/administrative skills, collaboration and empathy, an ability to work well independently and under high-stress, and commitment to healthy boundaries. A successful candidate will bring deep understanding of the unique Latino/a/x community, problem-solving and people skills as well as a strong ability to make decisions to ensure individuals and families are fully supported to consistently deliver high quality services with impact and excellence. 

Key Accountabilities: 

  • Ensure Spanish speaking clients are connected to THP programs and community resources 
  • Build and strengthen community partnerships 
  • Increase community awareness and understanding of THP services in Spanish speaking communities 
  • Collaborate with the Care Team Director to ensure all program deliverables are met and program evaluations are conducted 

Ensure Spanish speaking clients are connected to THP programs and community resources 

  • Provides peer education, care coordination, and support to Spanish speaking individuals and families  
  • Make referrals to partner organizations including health and human service organizations and follow-up with individuals to ensure they receive needed services 
  • Reduce stigma and other barriers to initiating or continuing to receive support by providing reliable information to both community members and providers 
  • Serve as a liaison between local health care and human service providers and the Network to ensure all team members have updated service information and to promote trusted relationships.  

Build and strengthen community partnerships 

  • Strengthen community partnerships with Latino/a/x serving organizations to connect clients with the necessary resources 
  • Develop relationships with local health care and human service providers to facilitate information sharing and service delivery.  
  • Aid with community connection and resources (maintain active up-to-date resource list) 
  • Work in collaboration with THP team members and external partners, communicate effectively with team, organizations and clinical providers 
  • Represents organization in meetings with community partners.  
  • Participates in advocacy coalitions or groups as interested or assigned.  

Increase community awareness and understanding of THP services in Spanish speaking communities 

  • Outreach to Spanish-speaking and other vulnerable populations to educate on and ensure access to THP programs including recovery, care coordination, and health insurance.   
  • Provide trainings about THP’s programs to Latino/a/x community members including our health coverage program, recovery program, and care coordination program, 
  • Schedule, promote, and attend Health Partnership events 
  • Supports THP marketing efforts by providing Spanish translation and culturally relevant insight. 

Collaborate with the Care Team Director to ensure all program deliverables are met and program evaluations are conducted 

  • Completes all required documentation including client documentation in electronic health record, internal programmatic tracking, monthly grant reporting, etc in a timely manner 
  • Works collaboratively across THP’s programs to ensure client needs are met 

*Other duties as assigned or identified through developing and ongoing work.  

General Requirements & Qualifications: 

  • Bachelor’s Degree or equivalent experience 
  • Must be fluent in Spanish and English and culturally sensitive to the needs of the Hispanic/Latinx community 
  • Health or Human Services experience preferred 
  • Engage in cross-organization efforts, connecting project work to the broader Partnership 
  • Share unique skills and expertise with THP team  
  • Work with clients, medical practices, community social service providers, complex medical patients and knowledge of health service delivery preferred  
  • Possess a Colorado driver’s license and have access to a reliable vehicle and be willing to travel throughout the counties, including during inclement weather 
  • Ability to communicate effectively with diverse audiences including clients, community members, professional partners, funders and government agencies 
  • Proficiency with Microsoft Outlook, Word, Excel, and PowerPoint 
  • High level of organizational skills with a focus on problem solving, detail oriented and follow through 
  • Active listening, motivational interviewing techniques, and the ability to support clients during intense emotional periods 
  • Commitment to inclusiveness, social justice, health equity and reduction of health disparity 
  • Identify and use data for data informed decision making, and to enhance collaborative work 
  • High tolerance for ambiguity and ability to problem solve and appropriate course of action 
  • Performs all other duties as assigned
